3. Your email address
4. Your GSM/CDMA phone number(s)
5. Your Liberty Reserve Account Name and Number
6. Your PayPal and/or AlerttPay Account ID if you have any (Optional)
7. A scan copy of your recent passport photograph (Not more than a month old)
8. A scan copy of your Government Issued Identity Card like International Passport, National ID Card or Driver's License (Expired documents will not be accepted).
9. A scan copy of your recent bank statement as Proof of Address. It must be duly signed and stamped by the bank and should not be more than 3months.
